MAPP Model
The National Association of County and City Health Officials (NACCHO)’s Mobilizing for Action through Planning and Partnerships (MAPP) model is used by county health departments to complete Community Health Assessments (CHAs). The MAPP model is a community–driven strategic planning process for improving community health, and its framework helps communities to apply strategic thinking to prioritize public health issues and identify resources to address them.
